What ACDF is.

Discs sit between the bones of your neck. When one wears down or herniates, it can press on a nerve root and cause arm pain, numbness, tingling or weakness. If it presses on the spinal cord, it can affect your balance or the fine movements of your hands. ACDF removes that disc from the front of the neck, which leaves the muscles at the back of your neck alone. A small cage filled with bone graft goes into the space the disc left, and a thin plate holds the two bones steady while they grow together into one. That joining is the fusion.

How the surgery goes.

A small incision at the front

Dr. Vemuri makes a short incision in a skin crease at the front of your neck and moves the soft tissue aside. No muscle is cut.

The disc comes out

The damaged disc and any bone spurs are removed, which takes the pressure off the nerve or the spinal cord.

A cage and a plate hold the level

A cage packed with bone graft fills the disc space, and a thin titanium plate holds it in place while the bone fuses.

When ACDF makes sense, and when it doesn't.

Most neck and arm pain gets better without an operation. Time, physical therapy, medication and sometimes an injection come first. ACDF is for the people that doesn't work for: arm pain, numbness or weakness from a disc pressing on a nerve, or signs that the spinal cord itself is being squeezed, after conservative care hasn't helped. Dr. Vemuri also does cervical disc replacement, so his recommendation is based on your anatomy and your imaging, not on the one operation he offers. If you don't need surgery, he'll say so.

The day of surgery

Many single-level ACDFs at miiSpine are done as outpatient surgery. You arrive in the morning and, if all goes as planned, go home the same day.

After

You get a written plan for activity, driving and work, and follow-up X-rays to check the fusion. Recovery is different for everyone, so the plan is yours, not a generic timeline.

What does ACDF stand for?

Anterior cervical discectomy and fusion. Anterior means the surgery is done from the front of the neck. Discectomy means the disc is removed. Fusion means the two bones are joined so that level no longer moves.

Is ACDF outpatient surgery?

Often, yes. Many single-level ACDFs at miiSpine are done as outpatient surgery, and some two-level cases qualify. Dr. Vemuri will tell you at your consultation whether you're likely to go home the same day.

How long does recovery take?

It varies from person to person, so we don't promise a number. Dr. Vemuri gives you a written plan for activity, driving and returning to work, and follow-up X-rays confirm the fusion as it heals.

Will I need a neck collar?

Some patients wear a soft collar for a short period after surgery. Whether you need one depends on your case, and Dr. Vemuri will tell you before surgery.

ACDF or disc replacement?

Both are done through the same small incision at the front of the neck, and both remove the damaged disc. ACDF then fuses the level. Disc replacement puts in an implant designed to keep that level moving. Dr. Vemuri does both, and the right one depends on your anatomy and your imaging.
